Dear Caregiver,
The Chenango Valley School District has chosen to participate in the Prevention Needs Assessment Survey (PNA). This survey of students in grades 6 – 12 is designed to improve our understanding of our children’s strengths, our community supports, and the risks they may face for problems like smoking, underage drinking, and drug use. The information the students provide can help us all take steps to reduce and prevent these risks and/or behaviors in the Chenango Valley School District and as well as the community. The survey is sponsored by the Broome County Mental Health Department in conjunction with the Prevention Coalition of Broome County, a group we are working with to build healthier communities.
The following are facts about the survey:
- It is totally voluntary. Your child does not have to participate. You do not have to give a reason for not allowing your child to participate. Also, students are not required to answer all questions, only those that they want to answer.
- It is anonymous. The survey is designed to protect your child’s privacy. No names or any other identifying information will ever be recorded.
- The Chenango Valley School District and the Prevention Coalition will use the survey results to plan better prevention efforts.
A copy of the survey is available at the Main Office of your child’s school for your review. You may also call me at 762-6900 for more information.
If you do not want your child to participate, simply sign and return the attached refusal form to the principal’s office by Monday, December 2, 2024. If you return the refusal form, your child will be given an alternative activity to do during the survey period.
Thank you for your help. This survey is an important part of Chenango Valley’s effort to keep our children healthy and safe. I hope you will allow your child to participate.
