This notice applies only to employees who take health insurance through the District. If you do not participate in CV-sponsored health insurance, no action is required.

In accordance with IRS regulations and the Paperwork Burden Reduction Act, the District is no longer required to distribute paper copies of Form 1095-C to employees.

Beginning with Tax Year 2025, employees who wish to receive a paper copy of their Form 1095-C may request one by emailing [email protected].

Paper copies will be mailed within 30 days of receiving your request. Please note that these forms will not be available before March 3, 2026, and requests cannot be fulfilled prior to that date.

This notice will remain available on the staff page of our website through October 15, 2026, as required by IRS guidance.
