We are very excited to welcome some new “CV Little Warriors” to our Port Dickinson Elementary School family. We will be registering students attending pre-kindergarten (4-year-old students) and kindergarten (5-year-old students) for the 2020-2021 school year. If you have a child who will be 4 or 5 years old by December 1, 2020, please click the link below to begin the registration process for your Little Warrior.

*It is important to note that if your child is currently enrolled in the Pre-Kindergarten Program at Port Dickinson Elementary for this current school year (2019-2020), they will automatically be enrolled for kindergarten, beginning in September 2020.

• Log back into the registration system and upload them in the screen for "Additional Info". Simply check the box for 'Online Registration Uploads' and you can upload files, including image files, directly to our system. If you need your access information to re-log into the online system, please email Sarah Latimer at [email protected]
  
• Stop by the security office at Chenango Valley Middle School (221 Chenango Bridge Road) and they will make the copies for you. Please be sure to wear a mask when visiting the security office.  

For new registrations and those who registered online prior to 6/24/2020, we have a couple more forms that we need filled out, but they are available online as Google forms. Please fill out the following:

• Housing Questionnaire 
• Home Language Questionnaire 
• Transportation Form
• Armed Forces Questionnaire

Finally, we will have some forms that we will mail home that you can either upload in the online system under the category of 'Other', drop off at the Chenango Valley Middle School/High School security office or mail back to us at 221 Chenango Bridge Rd., Binghamton, NY 13901 Attn: Registration Office. The forms to be mailed home are linked below in case you would like to print them off at home:

• Physical Exam Form
• Dental Form
• Free and Reduced Lunch Form
• Health History Form 

 Acceptable documentation for proofs of residency:

• Lease (needs to be notarized)
• Recent Utility Bill (heat, electric, phone, cable, water, garbage)
• Mortgage Papers
• Valid Motor Vehicle Registration
• Valid Driver's License (can be used as proof of residency and id)
