Please use this form when you have any changes in your child's tranportation. You can bring the completed form to the office of the building your child attends or bring it to the Transportation Office. Please note the cut off time to make transportation changes on half-days is 10 a.m. If you have any questions, please contact the Transportation Department at 762-6850.
